New Patient Registration
If you live in our practice area (see map) and would like to register with us, please ask at reception for a registration form or complete the registration form online and the pre registration questionnaire online. You will still need to call in to the surgery to sign the form after it has been sent.
You will be registering with the practice rather than an individual GP, although you will be able to say which practitioner you would prefer to see.
All new patients are offered a health check with a member of the healthcare team to ensure that any required tests are up to date and that we have an accurate note of any repeat medication you may be taking.
Medical treatment may be available from the date of registration, however an appointment to see a Clinician is only possible after the registration has been processed. Please contact reception for further information.
Please enquire at reception where you will be given the necessary forms to complete. You MUST provide your NHS Number to register with us. We also require some form of photographic ID and the person wishing to register must present the ID in person.
Patients requesting to join the Practice list will not be discriminated against on grounds of:
1. Race, gender, social class, age, religion, sexual orientation or appearance.
2. Disability or medical condition.
New patients will be asked to attend for a consultation with the practice nurse when registering with our practice. Please bring your list of medications with you to this appointment, preferably a print out from your previous Drs.
Sometimes it may take a while for your full medical records to arrive, therefore you may be asked some questions about your past and present health, and any treatment you are taking.
